STEP 2 - TECHNOLOGY AND INTERNET

Prior to your televisit, make sure you have a working smartphone, tablet, laptop, or desktop computer with a functioning camera, microphone, and speaker. You will need an adequate internet connection.

NO ADDITIONAL SOFTWARE NECESSARY FOR TELEMEDICINE VISIT.

STEP 3 - PRE-VISIT REGISTRATION

If your address, telephone and insurance is the same as it was at your last visit with Dr. Levine, go to STEP 4.
If there are any changes or you are a new patient, continue here:

• Copy your Photo ID.
• Copy the front and back of your Insurance Card (s).
• You can either make copy or take a smart-phone photo of the photo ID and insurance card (s).
• Please complete the Registration and Insurance forms.

STEP 4 - OUR STAFF WILL CONTACT YOU

Our staff will send you an email or text with your Internet link/ID that will allow you to join Dr. Levine for your appointment. You do not need any special software on your computer, tablet or smart phone.

STEP 5 - YOUR APPOINTMENT

At the time of your appointment, you will have received an email with the link to connect to your telemedicine visit.

CLICK THAT LINK. Dr. Levine will “admit” you for your visit.

Please be aware that your insurance company requires both the audio and video of the telemedicine visit be recorded. This is stored securely for your protection.
